TK/Kindergarten Enrollment

To protect the health of children, California law requires a health examination up to 18 months before or 90 days after your child's 1st-grade year. You will receive this form at the time of registration and will need to take it to your child's healthcare provider to be completed.

 Dental Health

To make sure your child is ready for school, California law requires that your child receive an oral health assessment by May 31st of their Kindergarten year. Assessments that happened within 12 months prior to school entry also meet this requirement.  A Kindergarten Oral Health Assessment form is included in the enrollment packet and will need to be taken to your child's dental care provider to be completed.
